Consent and Release

This Agreement informs you and you acknowledge that LIVE OFFICE HOURS videos are recorded. Your GIVEN CONSENT ON THE ENROLLMENT FORM CHECKBOX indicates that you understand that participation in LIVE OFFICE HOURS may reveal your name, your comments and your image and that you participate in these recordings with full knowledge and full consent.

I, hereby irrevocably and perpetually grant to Pure Body Systems LLC dba The American Institute of Integrative Oncology Research and Education (AIIORE) (“COMPANY”) its subsidiaries, affiliates, successors and assigns:

The right to videotape and record my name, voice, appearance, likeness, and/or written testimony along with any material furnished by me, in whole or in part, during Company’s Live Office Hours with Dr. Nalini, in any of COMPANY’s books, program and marketing materials and programs.

The right to use my written, oral and video submissions in any fashion COMPANY uses, in its discretion.

The right to publish, exhibit and distribute the use of my name, voice, appearance, testimonial and/or likeness along with any material furnished by me, in whole or in part, worldwide, for any commercial purpose, including but not limited to education, advertising or solicitation of business, by any means of mass and/or electronic media, including but not limited to print, radio, television and promotional materials, events and/or marketing plans.

I understand COMPANY is relying on this Consent and Release by recording my appearance and that my authorization and this Consent and Release is irrevocable and may not be withdrawn. I waive any right to inspect any materials of any time prior to release, use or publication, and, on behalf of my heirs, executors and assigns, I waive any claims that I may have against COMPANY, its representatives, its publisher, and their licensees, successors and assigns, based upon such use, including without limitation any claims with respect to defamation, rights of privacy and publicity and copyright.

I hereby acknowledge that I have not been paid or otherwise compensated, and I further certify that all material, whether verbal, written or exhibited by me has not been scripted and represents my individual opinions and beliefs, and is true and correct to the best of my knowledge.

RENEWAL:

This Agreement shall, commence on the date of this Agreement (Purchase date) and shall continue for the period specified in the order form and, thereafter, this Agreement shall be automatically renewed for successive periods on a monthly basis, unless:

a) either party notifies the other party of termination, in writing, no less than 30 days before the end of the Initial Period or the end of any Renewal Period, in which case this Agreement shall terminate upon the expiry of the applicable Initial Period or Renewal Period as applicable.
